This essay should showcase your specialised area of knowledge and understanding of how care is co-ordinated for complex health needs in a Child, Adult or Mental Health case.
You should select a patient which represents this group from scenarios created within the module.
Explain why their health needs require long term health interventions/inputs to demonstrate your understanding of the importance of partnership, collaboration and interagency working.
You are required to analyse the relevant policy and legislation in relation to care co-ordination and critically evaluate the extent to which their care is co-ordinated.
Your work should have a strong evidence base, include references from a wide range of sources, recognizing the complexities and alternative approaches in the literature.
