The need to investigate and address the causes of even minor events should not be underestimated and it is essential to focus not only on what actually happened,but on the potential consequences. Several institutions such as Health and Safety Executive (HSE) investigate and publish reports on major accidents, emphasising the lessons to lean for related industries.
Similarly the UK Institution of Chemical Engineers also publishes a ‘Loss Prevention Bulletin’ that focuses on incidents and lessons for the wider industry whereas the Chemical Safety Board (CSB) in the US carries out investigations into major incidents. Companies use all this external information alongside their internal data on incidents as ‘lessons learnt’ in the continual improvement process.
You as a team are tasked to collect information about at least five past incidents involving companies of your choice (do not include examples used in class such asPiper Alpha, Buncefield fire and Gulf of Mexico BP oil disaster). You are required to: select incidents from different industry sectors (energy, oil and gas, mining, manufacturing, food,pharmaceutical,….).
Critically review these past incidents under the six functional elements (knowledge and competence, engineering and design, systems and procedures, assurance, human factorsand culture) of an organisation as defined in the framework proposed by the I ChemE Safety Centre for viewing process safety across an organisation.
Include in your review:-a table of immediate and root causes, impact and any legal requirements that were not met at the time of the incident);-any measures, disciplinary actions or revised risk assessments undertaken by the company involved;-Any potential emerging risk include in the individual report:-a reflection about your key learning points or knowledge gained by carrying this assignment;-a completed self-assessment form (University Performance Descriptors, Level 6).
