SWOT Analysis and Community Problem Statements
Muecke (1984) describes goals of community health nursing in terms of primary prevention to decrease disease and threats to health. A community health nurse performs comprehensive windshield, health profile and demographic assessments to identify challenges the community’s members face that poses a threat to their health as a whole.

Data of interest includes but is not limited to disease and injury rates that require members to seek health care. Sources of data include hospitals, primary care clinics, 911 system, and the health department. The strengths, weaknesses, opportunities, and threats (SWOT) framework will be applied to analyze opportunities for improvements and Muecke’s framework will be used to specifically state two high-priority community problems.

SWOT Analysis
SWOT analysis for Census Tract _______ contains four categories: 1) strengths, 2) weakness, 3) opportunities, and 4) threats. Internal factors include strengths and weaknesses of the community, while opportunities and threats are factors from outside of the community. Diagnosis 1

Increased risk for heart diseases among adults 65 years and older related to comorbidities, lack of health maintenance, and poor diet consumption demonstrated by the rise of heart diseases from 21.8% to 24% in the county and 22.4% to 22.8% in the state (Texas Health Data, 2012 & 2015), increase of Kidney Disease from 2% to 2.1% in the state (Texas Health Data, 2012 & 2015), the unchanged proportion of Diabetes (Texas Health Data, 2012 & 2015), the substantial availability of fast-food, inability to access healthcare from lack of funds due to the high numbers of families living below poverty of 123 in Census Tract _______ and 54,320 families in Bexar County (U.S. Census Bureau, 2015 & 2018), and the increased rate of those with education less than 9th grade in Census Tract ______ by 2.9% (U.S. Census Bureau, 2015 & 2018).

SWOT Analysis and Community Problem Statement Summary

The ability to identify the strengths, weaknesses, opportunities, and threats in a community through data collection from the Windshield Assessment, Texas Health Data, and the United States Census Bureau would assist the community health nurse to identify opportunities for improvement that will increase the overall health of the population.

Two priority nursing diagnosis have been determined using Muecke’s framework. These diagnoses include information relating to the potential risks, the population that is at risk, factors that contribute or relate to the risks, and measurable evidence related to the risk (Muecke, 1984).

Now that assessments and diagnosis have been completed, the nursing process will be applied to create a plan for action including a goal, related interventions and outcome measures.
