Given two clinics discussed by Lutfey & Freese (2005), evaluate the differences in lifestyles, SES, and accessing care through diabetic health outcomes demonstrated in a diagram.
Create two diagrams by identifying and distinguishing the differences in SDOH (Social Detriments of Health) for each of the clinics with proficiency achieved with the identification of at least 4 SDOH identified per clinic (3 SDOHs need to be related to factors outside of the clinics and 1 SDOH needs to related to factors internal to the clinic (provider expertise, training, type of physicians).
Indicate at least two ways SES (Socioeconomic Status) influences differences in health outcomes for the two clinics (100 words). B. Indicate at least two ways healthy lifestyles are influenced by SES for the two clinics (100 words). C. Indicate at least two ways health outcomes are influenced by both SES and healthy lifestyles for the two clinics (100 words).
Develop two solutions or interventions to improve health outcomes for diabetic patients (one for each clinic). (50 words per clinic)
