Medication indications and specification
Each medication should have their own paragraph with the following
1) Eliquis 2.5 mg twice daily (blood thinner)
2) Diltiazem 24 hour capsule 240 mg by mouth ( hypertension/chest pain)
3)Colace (docusate) 100 mg capsule by mouth twice daily (stool softer)
4) Gentamicin 50 mg IVPB every 12 hrs ( antibiotic used for patient with MRSA)
5) Metoprolol 50 mg tab by mouth every 12 hrs
6) pantoprazole (protonix) PUMP- 40 mg twice a day use for GERD
7) VANCOMYCIN 1.5 g IVPB ( treatment of infection-MRSA)
including name, dose, frequency; class and mechanism of action; what condition it is being used to treat; major contraindications and side effects; nursing implications; and client teaching points specific to the client for each medication.
