on 12th November 2020, and investigated by the UK AAIB. Conduct a Hazard Analysis based on the evidence presented in the report, review the hazards that might be evident, list the possible hazards for further analysis and produce a Bow Tie for the most significant hazard identified.

Note: This question has two parts, the hazard analysis is worth 10 and the Bow Tie is worth 20. (30%)
