Scenario from practice:
James, the anonymous name used for privacy and confidentiality, is the patient identified on placement. He was referred to the community mental health team for MSE by his GP. The mental health team arranged a telephone triage but failed due to James’s noncompliance by not answering his phone calls. As a result, a home visit was arranged, and two nurses attended. James home for his mental state assessment. According to care notes, all signs of symptoms of self-neglect were observed at James home, which led to raising self-neglect safeguarding. It was also observed that he excessively used alcohol, did not attend to his nutrition needs, medication was all-over an indication that he was not very and personal hygiene which raised concerns. In addition, he told the nurses that he was suffering from depression and suicidal ideation. James attempted to commit suicide in the presence of the nurse’s police was called James was put on Section 136 and admitted to the hospital.
