S: Ms. Oxendine is nineteen years old with Hx of despair and an attempt of committing suicide with the use of 7500mg of “acetaminophen oral poisoning” presently in good conditions and pending “AOx4. VSS.” lab test.

B: the patient attempted suicide as a result of a breakup with her boyfriend. She stated that she just commenced her college education and her grades are not good enough and hardly have lots of friends. Despair because she is aged fifteen years is on treatment. A past attempt of committing suicide in high school at an unknown age.

A: AOx5 is presently on the patient’s assessment. There is tiredness, the patient states a lack of interest in normal activity and she as well states that she doesn’t have an appetite and lucks sleeps. Her skin is warm with the presence of L PIV.

R: keeping prescribing systematic meds whereas inpatient apiece of MAR. Suicide precaution gave with psychological consultations.
