Problem: In order to facilitate early care coordination for homeless individuals, the social determinants prior to discharge, the EHR requires the ability to document and extract discrete data from the EHR by allowing nurses to document defined social determinants of health during the nursing assessment upon admission.

Intervention: Create a ‘social determinants of health’ field that will require nursing staff to complete homeless status upon the initial assessment. The expectation is for nurses to document problems concerning social issues such as homelessness.
